Liability & Release

Assumption of Risk and Waiver of Liability

1. Voluntary Participation
I understand and acknowledge that participation in activities organized by the Vegreville
Lasers Swim Club, including but not limited to swim practices, meets, training sessions, and
related events, involves inherent risks, including the risk of serious injury, illness, or death.

2. Assumption of Risk
I voluntarily assume all risks associated with participating in Vegreville Lasers Swim Club
activities, whether those risks are known or unknown, and accept full responsibility for my
participation or that of my minor child.

3. Release of Liability
To the fullest extent permitted by law, I hereby release, waive, and discharge the Vegreville
Lasers Swim Club, its coaches, officers, directors, volunteers, agents, and representatives
(collectively, the "Released Parties") from any and all liability, claims, demands, or causes of
action arising out of or related to any loss, damage, injury, or death that may occur to me or
my child while participating in swim club activities, including travel to and from training
sessions, swim meets or other scheduled team activities, whether caused by the negligence
of the Released Parties or otherwise.

4. Medical Treatment Authorization
In the event of an emergency, I authorize the Vegreville Lasers Swim Club to seek
emergency medical treatment as deemed necessary. I understand that I am responsible for
all costs associated with such treatment.

5. Acknowledgment and Agreement
I have carefully read this waiver and release of liability, fully understand its contents, and
sign it voluntarily. I understand that I am giving up legal rights, including the right to sue the
Vegreville Lasers Swim Club and its representatives.
